Registration PoliciesEarly registration is encouraged and accepted on a first-come, first-serve basis. If there are registrants who cannot be confirmed due to high demand, they will be placed on a waiting list. The Registration Form is available at this Web Site for you to fill in and print or email direct. Please see the specific instructions on the registration page. We also offer walk-in registration at the door when space is available. Payment Policy:The full fee is required with your registration. We accept Visa, MasterCard, American Express, purchase orders, certified checks and money orders. Pricing, registration terms, and conditions are subject to change without notice. Registration fee covers all course materials and your Certificate of Completion. Course Locations: : Locations for our courses vary depending on the course and date. Be sure to carefully review the course schedule and select the course which is at a location and date convenient for you. If a particular course is not being offered in your geographic area during a specified time, please contact us at 1-888-436-1814 as we maintain a “wish list” and will add courses according to demand. Confirmation: Upon receipt of your registration, electronic or U.S. mail confirmation will be sent verifying your seat in the class or your wait list status if applicable. Course Schedule: Classroom sign-in and on-site registration begins at 8:30 a.m. on the first day. Courses normally start at 9:00 a.m. and conclude at approximately 4:30 p.m. each day. However, please verify the times as indicated on your confirmation notice as some class hours may vary. Registrant Withdrawal and Substitution Policy: If a registrant is unable to attend a class UMI must be notified in writing at least seven business days prior to the class. Course credit or a full refund of the registration fee will be issued. The notification can be completed by fax to 321-385-1586 or email at training@UpperMohawkInc.com. Because of costs incurred, withdrawals received after seven business days prior to the class start date will not be eligible for a refund. Substitutions are welcome up to seven business days before the class start date with a completed registration form for the substitute. Course Cancellation: If Upper Mohawk, Inc. is forced to cancel or postpone a course due to insufficient enrollment or unforeseen circumstances, liability is limited to the registration fee only. Registrants will be notified by phone or electronic mail. The registration fee may be applied toward future courses or a full refund will be issued. Americans with Disabilities Act: When registering by phone, mention any special accommodations you require in order to attend the courses listed in this catalog. When registering by mail or fax, please attach a letter with your registration form explaining your needs. Course Evaluations: Because UMI strives to provide quality training opportunities, trainees will be asked to complete a course evaluation upon completion of all courses. Responses are reviewed by UMI Training and Development management staff and recommendations are considered and valued. Customized Training: : UMI can customize existing curriculum or develop a unique curriculum to meet your organization’s specific needs and schedule training when it is most convenient for you.
